A. The solution to your scarring problem may be difficult for several reasons. One being the age of your scars and the second is the lack of definite knowledge and protocols to remedy such scarring. You seem as though you really want to clear your scarring problem up, but we cannot guarantee any of the suggestions I am about to make will work. But these are the two approaches I would suggest First-. Try a combination of the ReJuveness silicone sheeting and Hyper-Heal cream. This would be a protocol which entails the wearing of the silicone sheeting at least eight hours a day while putting the scar cream underneath. You would also Hyper-Heal cream when you are not wearing the sheeting. The cream should be used very sparingly and rubbed in gently. If you don't start to see results in a month you may want to consider the second option below. If you do see results keep using this simple protocol. If you are lucky It may take up to nine months to completely solve the problem. The second approach would be to re insure the site by ablation. This can be done with chemical peals or any other method that removes the stratum corneum or top layer of skin. you would then use the ReJuveness silicone sheeting immediately after. If you choose to do this you should get a professional medical esthetician or dermatologist or plastic surgeon involved. The problem we are trying to overcome is that there is a memory to your skin. That memory are always pushing up the scars of your old injury. Although not originally indicated for these indented scars, ReJuveness silicone sheeting has solved their problem of indented scarring resulting from acne. But it doesn't work overnight or even a week. You may be wearing ReJuveness sheeting on a nightly basis for five six maybe even a year before your problem is resolved. A. Unfortunately I was unable to make out the appearance of the scar from the picture you sent. It came out of focus and perhaps to big. Unfortunately the world of scar management and wound management remains confused at best at this moment. The healing of a wound and scar formation is a very complex state involving healing cascades and signal transduction driven not only a person's genetic constitution but also the the chemical constitution and the particular state of the bio-molecules present. I can give our viewpoint of the treatments you tried so far and how ours may differ, but keep in mind that when trying to achieve a near scar less state timing and early intervention are important. First of all the type and brand of silicone sheeting can make a big difference in scar management. Because many of the healing cascades are believed to be immune responses involving cytokines, mast cells, mono cytes , macrophages, surface cell receptors ect. they may be responding to silicates in a signal transductive manner. So early wound phenomena like redness, itching, swelling are the same response as say a mosquito or spider bite. These immune response molecules are produced and carried by the blood which have their origin in the bone marrow which is silicon based. This may account for a sensitivity of these immune response molecules to silicates in their different forms. So a silicone gel sheeting like Cica-Care that you used on your daughter's burn scar may act very different than a elastomer sheeting like ReJuveness which has a more brittle reaction to hydrolization and release of silicates due to moisture from the skin hotting the top layer of the sheeting. In a test we did we found that ReJuveness was releasing 200 times more tiny particulate silicates (probably negatively charge) than cica-care and other gel-sheeting. But a simple explanation is that the silicate s are released and they stimulate the macrophages, which are probably laying dormant in these problem wounds, which then initiates the release of CO2 which is the key to much of the starting of healing cascades. The silicone also attracts lipids to itself which is followed by calcium seeking the lipids. There is probably almost a slight simulation of bone formation at the surface of the skin using silicone sheeting. The silicone gel sheeting molecules released are macro-molecules which have a much different response. In biopsies done after using Cica-care and ReJuveness sheeting there were traces and evidence of silicone remains locally in the Cica-Care , where with the ReJuveness no evidence of silicates remained in the scar. There fore ReJuveness not only being more effective and releasing more silicates was also safer because no silicate residues remained. As far as the creams go, I am assuming the cream you mentioned was some sort of cortisone based cream. These simply do not work because there is a certain threshold of cortosone needed to resolve problem scars like keloids. Several series of kenolog injections are needed for these treatment strategy to work. The levels of cortosone in these creams does nothing and dosen't unnecessarily translate into a wound healing strategy. The high levels of cortisone are thought to destroy the fibroblast which produces the collegen making scar tissue. But this destruction of fibroblast is complete, sheath and all. Unfortunately these fibroblast are needed in other aspects of wound healing namely the production of collegenase an enzyme which digest collegen and reduces scar tissue. On the other hand The Hyper-Heal cream contains different types of ceramide which is known to effect the kenase C healing cascade , which control the apoptosis of fibroblast. It theoretically does the same thing as the kenolog shots but in a more gentle non-invasive way of signal transduction. It is like comparing turning off a light in a room by blowing the room up (kenolog injection) or turning the light of by switching the light switch off (Ceramide signal transduction). But anyway I hope this kind of answers your questions which are probably much more complex than you think. The bottom line is that the cause and effect in healing wounds is very rarely a direct relationship of the medications we are applying. the most successful methods now involve trying to influence healing cascades through signal transduction. We at ReJuveness are pursuing this goal by employing the least toxic materials we can find that will have the biggest impact. We still have a very long way to go. A. What you describe is normal progression. Try to put a little lotion on the area where the tape is irritating. Even something like baby oil will work fine. You can cut and shape the piece so it custom fits the area you are addressing. treatment time is usually 6 weeks to 3 months but these pitted scars can take longer.
